Please fill out the application form provided at the question signs or write your name, zip code, address, and answers to all three questions on a plain, postcard-size sheet of paper.
Please fill out the form and put it in the application box located on the bulletin board of the shopping mall office.
■Only one application per person, please.
■Official postcards may also be submitted.
